GRADE RELEASE

I, the undersigned, understand that state and federal law, including the Family Education Rights and Privacy Act of 1974 (as amended) prohibit the release of my scholarship records without my consent (absent other exceptions). I hereby give my consent to the UO records officers to release my grades to the president or scholarship chair of my chapter for the purpose of participating in chapter scholarship activities. This consent is valid for the duration of my association with the UO fraternity and sorority community or until I file a written revocation, whichever comes first.

Records may be released to chapter leadership, chapter advisors, and chapter inter/national headquarters staff and volunteers. My individual GPA will not be publicly released, but it will be factored in to a publicly released statistical document for chapter ranking. The purpose of the disclosure is for use in chapter/council scholarship, general statistics, standards, and educational programing, award recognition, and verification of minimum academic standards and University enrollment.

HAZING AGREEMENT

I, the undersigned, have read UO Student Conduct Code, and the UO statement of policy regarding hazing. I have also read and understood the policies of the governing council (IFC, PHC, MSFC, and NPHC) and my own chapter that pertain to hazing. I understand that hazing is defined as any initiation rites, recruitment and continuing involvement and belonging to an organization on or off campus, involving any intentional action or situation that a reasonable person would foresee as causing mental or physical discomfort, embarrassment, or ridicule. Individual acceptance of or acquiescence to any activity that occurs during an initiation rite does not affect a determination of whether the activity constitutes hazing. I expect my chapter to abide by the policies and regulations as is agreed to in the President’s Anti‐Hazing Agreement and with this Membership/New Member Agreement.

FSL ADMINISTRATION FEE

By signing this agreement, you commit to paying the annual UO FSL Administrative Fee for the entirety of your membership. The $125 fee will be billed as a part of your student fees/tuition once per academic year. 

MODEL RELEASE

I, the undersigned, irrevocably grant the University of Oregon Fraternity and Sorority Life permission to publish, republish, adapt, exhibit, reproduce, modify, make derivative works, distribute, or display my name, image, voice, written testimony, and biographical information in connection with any university product or service. This permission applies to all markets and in any media or technology now known or hereafter developed. The university may exercise any of these rights itself or through any commercial or nonprofit successors, transferees, or licensees.

I waive any right to inspect or approve any work that bears my name, image, voice, written testimony, and biographical information.

By typing my name and the date below, I certify that (i) I accessed this Anti-Hazing and Grade Release using my unique University of Oregon Duck ID account which authenticates my identity, (ii) I am the individual identified in the Agreement and Release, and (iii) I accept and agree to this Agreement and Release as written as if signed with my own handwritten signature.
